Basic information on health insurance for TUL students and doctoral students who are not Polish citizens



 

I. A student / doctoral student who is not a citizen of a European Union Member State or a European Free Trade Association (EFTA) Member State and is not subject to compulsory health insurance in the territory of the Republic of Poland and in another European Union Member State or European Free Trade Association (EFTA) Member State may insure himself voluntarily concluding an agreement with the National Health Fund, if he resides in the territory of the Republic of Poland.

The contract is concluded on the basis of a written application and a certificate confirming the student's status and passport. A student who concludes a contract and pays contributions in a timely manner is entitled to a full range of healthcare services in Poland in facilities that have concluded a contract for the provision of medical care with the National Health Fund. Within 7 days of signing the contract, the student is obliged to register it with the Social Insurance Institution [ZUS] by submitting the ZUS ZZA form. The contract premium should be paid each month to the account indicated by the ZUS, no later than until the 15th day of the following month. The contract is concluded for an indefinite period. The contract is terminated on the basis of a written notice or after a month of uninterrupted arrears in paying contributions or the creation of an obligation to compulsory health insurance, e.g. conclusion of an employment contract. The document confirming coverage with health insurance is a copy of the contract and a proof of payment of the premium for the last month. The right to health insurance benefits begins on the date of insurance coverage specified in the Contract and expires after 30 days from the date of termination of health insurance in the National Health Fund.

 

II. Student / doctoral student who is not a citizen of a European Union Member State or a European Free Trade Association (EFTA) Member State and is not subject to compulsory health insurance in the Republic of Poland and in another European Union Member State or European Free Trade Association (EFTA) Member State, recognized as a person of Polish origin within the meaning of the provisions on repatriation on the basis of a consul's decision or with a valid Polish Charter may voluntarily insure himself by concluding a contract with the National Health Fund, if he resides in the territory of the Republic of Poland. Health insurance premiums are paid by the University after completing the ZZA form and the declaration and submission of the contract made with the National Health Fund (NFZ). The form with the declaration and the contract made with the National Health Fund should be submitted to the Student Mobility Section of TUL in room 113 of the building of the International Cooperation Centre, at ul. Żwirki 36, building A16 or the Services Section at ul. Żeromskiego 116, building of the Faculty of Materials Technology and Textile Design, staircase E (from Radwańska Street), 4th floor.

 

III. A student / doctoral student who is a citizen of a European Union Member State or a European Free Trade Association (EFTA) Member State is entitled to free healthcare services provided by healthcare providers who have concluded contracts for the provision of healthcare services with the National Health Fund on the basis of the European Health Insurance Card or - if he is not entitled to insurance in the country of residence - he is reported for insurance by the University on the terms applicable to Polish students.
